Our team does everything possible to connect you with the most appropriate clinician to meet your child’s and family’s needs. First, we conduct a brief intake screening to match you with the needed clinical services.

For your first visit: After you have completed your intake screening and have scheduled an appointment, you will receive a packet of information which includes several questionnaires. Please complete these forms and return them to us as soon as possible along with copies of the following:

• Previous psychological, developmental or academic evaluation(s), recent report cards, and standardized testing results.


• Recent individualized educational plan (IEP) or 504 Accommodation Plan


• Recent work samples of your child’s written language (in their own handwriting) and math.
